If the user have a microphone adapter, he can use the app to turn the device into an cell Phone.

Using the microphone adaptor, Truphone for iPod touch lets you make free calls to other iPod touch owners, other users of Truphone’s Internet telephony service, and to users of Google Talk, no matter where any are in the world.

More features like Making and receiving low-cost calls to and from landlines at low cost (simply set yourself up with a Truphone account) and Calling and instant messaging to Skype and MSN, Checking and setting facilities for Facebook, Checking and setting facilities for Twitter, all three later services for free, seems to get added to the feature list of the app very soon.
